Basic Information On Procoralan
Procoralan is used in patients with coronary artery diseases but normal sinus-rhythm. In these patients the drug is used in the management of chronic stable angina pectoris which is treated symptomatically. The drug is used to treat patients who are contraindicated or not able to tolerate beta-blockers. The drug is also used as a combination therapy in patients who are not controlled with optimal doses of beta-blockers alone and who have heart rate greater than 60 beats per minute (bpm). The mechanism of action of Procoralan is different than that of the calcium channel blockers and beta-blockers. The drug acts on the most important ion current at the SA node, which is responsible for the regulation of pacemaker activity. Brand or generic Procoralan selectively blocks this pacemaker current and reduces its activity. This helps in reducing the heart rate, which allows extra time for the blood to reach the myocardium.
Precautions / Side Effects For Procoralan
The common side effects of Procoralan are bradycardia (low heart rate), ventricular extra-systoles, AV block, blurred vision, luminous phenomena, dizziness (probably due to low heart rate) and headache (usually in the 1st month of the treatment). Uncommon side effects of the drug include hyper-uricaemia, increased creatinine level in blood, eosinophilia, constipation, diarrhea, nausea, dyspnoea, muscle cramps and vertigo. The drug should be administered with caution in patients with prior history of atrial fibrillation, cardiac arrhythmias, hypotension, mild to moderate hepatic insufficiency and severe renal insufficiency. Avoid Ivabradine if you have the following: sinus syndrome, acute MI (myocardial infarction), cardiogenic shock, heart rate less than 60 bpm (resting), severe hypotension (less than 90/50 mmHg), sino-atrial block, pacemaker dependency, unstable angina, 3rd degree AV block and heart failure. Pregnant women should avoid the use of brand or generic Procoralan.
Procoralan Dosage
The starting dose of Procoralan drug is recommended to be 5 mg administered twice a day. The drug is administered orally in form of tablets. The response of the patient is observed over a period of 3 to 4 weeks. Based on the therapeutic response and tolerability of the patient the dose may be increased to 7.5 mg, taken twice daily. The dose is reduced to 2.5 mg twice daily (that is half of 5 mg tablet taken twice daily) if the heart rate is as low as 50 bpm. The treatment with Procoralan drug should be discontinued if the heart rate is below 50 bpm.
